Creighton University School of Dentistry is seeking a motivated individual to work with a team of dedicated faculty and
leaders who are building on a rich legacy of excellence in dental education and expanding our capacity as a national
resource for the dental profession. Position responsibilities include working with faculty, staff, community partners,
students and patients to facilitate timely and appropriate care in a holistic and compassionate manner. The Coordinator
will have experience in the social services field and is anticipated to serve as a liaison between the School of Dentistry
and the community rotation sites. Candidates must also possess strong organizational and communication skills with
an ability to work collaboratively in a collegial, interdisciplinary environment. Creighton University is a Catholic and

Omaha, Nebraska. Knowledge, Skills, Abilities: Ability to work collaboratively with a healthcare team to assess patient
psychosocial needs and support systems, provide referrals to community resources, and follow-up with patients as
needed through case management. Capacity to increase patient outcomes by addressing barriers to patients’ oral
health care such as transportation, financial limitations, insurance issues, language, and personal safety. Addressing
these barriers to care will help to reduce missed appointments and noncompliance, thereby increasing patient continuity
of care. Proficiency in electronic health records for documentation of patient interactions and must be comfortable using
telehealth as a service delivery method.

Bachelor's Degree
1-2 years experience in community outreach activities

Knowledge, Skills, Abilities:
Great customer service skills and listening skills.
Organization a must.
