Qualifications:
Bachelor's Degree
1-2 years experience in community outreach activities

Knowledge, Skills, Abilities:

Ability to work collaboratively with a healthcare team to assess patient psychosocial needs and support systems, provide referrals to community resources, and follow-up with patients as needed through case management. 

Capacity to increase patient outcomes by addressing barriers to patients’ oral health care such as transportation, financial limitations, insurance issues, language, and personal safety. Addressing these barriers to care will help to reduce missed appointments and noncompliance, thereby increasing patient continuity of care.

Proficiency in electronic health records for documentation of patient interactions and must be comfortable using telehealth as a service delivery method.
